How to create and use a custom form

Overview

When you need to collect information from students, parents or even visitors to your school website, you can use a custom made form.

Administrators can easily create forms using TeachHero's block-based form builder.

Forms can then be used as standalone forms by sharing a URL to them; used in student applications; or sent directly to families to request information.


How to Create a New Form

  1. Navigate to the forms page
  1. Click Add form
  1. Give your form a name (this is seen publicly as the form title)
  1. Click create

Your form is created in a draft state without any fields. Let's fix that by continuing to edit our form.


How to Edit a Form

  1. View the form
  1. Click Edit form

Adding form fields

  1. Click the Add field button
  1. Select the type of field you want to add
  1. Fill out all required fields to customize the form field
  1. Click add to add it to the form

Don't forget to click Save changes after making changes to the form or adding and removing fields.

Form Settings

Click on the Settings tab to make other changes to the form. You can change the form's name, the submit button label, edit the submission message show after a user successfully submits the form, or have users redirected to a URL after completing the form.

Sharing Forms with Teachers.

If this form is associated with a student and you want teachers to be able to view the information that was submitted, make sure to the check the box Share with teachers on the form settings page.
